What Is a Drug Interaction?

A drug interaction occurs when two or more substances (drugs, foods, alcohol, or supplements) affect each other's activity in the body. The result can be that a drug becomes more powerful, less powerful, or creates new effects that neither substance would cause alone.

Types of drug interaction

Pharmacokinetic interactions

These affect how the body processes a drug โ€” how it is absorbed, distributed, metabolised, and eliminated. The most common mechanism involves liver enzymes (particularly the cytochrome P450 family). If one drug inhibits or induces these enzymes, it changes the blood levels of another drug. For example, fluoxetine inhibits CYP2D6, which is why it makes tramadol both less effective and more toxic.

Pharmacodynamic interactions

These occur when two substances have additive or opposing effects on the same body system. For example, ibuprofen and alcohol both irritate the stomach lining โ€” together the damage is greater than either alone. Similarly, diazepam and alcohol both depress the CNS โ€” together they can stop breathing.

Drug-food interactions

Some foods affect drug metabolism. Grapefruit juice inhibits CYP3A4, potentially raising the levels of many drugs including statins. Vitamin K in green vegetables affects warfarin. Dairy products bind to tetracycline antibiotics (like doxycycline), reducing absorption.

Why "generally safe" still needs care

Individual variation means that "generally safe" does not mean "safe for everyone." Kidney function, liver function, age, genetic variation in drug metabolism enzymes, and dose all affect whether an interaction becomes clinically significant.
