AVOID THIS COMBINATION

Avoid โ€” combining sertraline and tramadol carries a serious risk of serotonin syndrome, a potentially life-threatening condition.

Sertraline also sold as: Zoloft, Lustral ยท Tramadol also sold as: Ultram, Tramacet (with paracetamol)

Is it safe to take Sertraline with Tramadol?

Sertraline and tramadol should not be taken together without explicit medical supervision. Both drugs increase serotonin levels in the brain through different mechanisms. When taken together, serotonin can accumulate to dangerous levels โ€” a condition called serotonin syndrome. Symptoms can range from tremor, agitation, and diarrhoea in mild cases, to seizures, high fever, irregular heartbeat, and muscle breakdown in severe cases. This combination also reduces the pain-relieving effectiveness of tramadol and increases seizure risk.

How do Sertraline and Tramadol interact?

Sertraline inhibits the serotonin transporter (SERT), preventing serotonin reuptake and raising synaptic serotonin. Tramadol also inhibits serotonin (and noradrenaline) reuptake, as well as being an opioid receptor agonist. Sertraline also inhibits CYP2D6, the enzyme responsible for metabolising tramadol into its active opioid form. This means less tramadol reaches its active opioid form, reducing its pain-relieving effect while its serotonergic effects remain โ€” a combination that is both less effective and more dangerous.

Who should be careful?

Anyone taking any SSRI or SNRI antidepressant should be extremely cautious about tramadol. If you need strong pain relief while on sertraline, discuss alternatives with your GP. This interaction can affect anyone, not just those with known risk factors.

What if I've already taken Sertraline and Tramadol?

If you have taken both together and develop agitation, confusion, rapid heart rate, muscle twitching, fever, or diarrhoea, seek emergency medical help immediately. Serotonin syndrome can develop rapidly. Call 999 if symptoms are severe.

Frequently Asked Questions

Can sertraline and tramadol be taken together?

No โ€” this combination carries a serious risk of serotonin syndrome. Always tell your doctor about all medications you take before being prescribed tramadol.

What is serotonin syndrome?

Serotonin syndrome is a potentially life-threatening condition caused by excess serotonin in the nervous system. Symptoms include agitation, confusion, rapid heart rate, muscle twitching, high temperature, and in severe cases, seizures and death.

What painkillers are safe with sertraline?

Paracetamol is generally safe. Ibuprofen or naproxen can be used short-term but carry a slightly increased bleeding risk with SSRIs. Opioids other than tramadol (such as codeine) carry lower serotonin syndrome risk but should still be discussed with your doctor.

Can I take tramadol if I have recently stopped sertraline?

You should wait at least 2 weeks after stopping sertraline before taking tramadol. Sertraline's effects on serotonin can persist for some time after the drug is stopped.
