Is it safe to take Sertraline with Tramadol?
Sertraline and tramadol should not be taken together without explicit medical supervision. Both drugs increase serotonin levels in the brain through different mechanisms. When taken together, serotonin can accumulate to dangerous levels — a condition called serotonin syndrome. Symptoms can range from tremor, agitation, and diarrhoea in mild cases, to seizures, high fever, irregular heartbeat, and muscle breakdown in severe cases. This combination also reduces the pain-relieving effectiveness of tramadol and increases seizure risk.
How do Sertraline and Tramadol interact?
Sertraline inhibits the serotonin transporter (SERT), preventing serotonin reuptake and raising synaptic serotonin. Tramadol also inhibits serotonin (and noradrenaline) reuptake, as well as being an opioid receptor agonist. Sertraline also inhibits CYP2D6, the enzyme responsible for metabolising tramadol into its active opioid form. This means less tramadol reaches its active opioid form, reducing its pain-relieving effect while its serotonergic effects remain — a combination that is both less effective and more dangerous.
